Please download the attachment pertaining to Good Faith Estimates.
We will provide additional required forms directly to you prior to the first appointment.
As of January 1, 2022, federal healthcare laws have changed to protect consumers against unexpected medical charges/bills.
You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost
Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.
**IF YOU DO NOT HAVE OR DO NOT PLAN TO USE INSURANCE, OR IF WE ARE OUT OF NETWORK, WE WILL PROVIDE YOU A GOOD FAITH ESTIMATE PRIOR TO YOUR FIRST APPOINTMENT**
We encourage you to bring concerns about any aspect of your care directly to your therapist so they may be addressed. Alternatively, Dr. Chris Howells (Owner/CEO and clinical psychologist) is available to address any business-related concerns and/or clinical concerns that cannot be directly addressed with your therapist. Many concerns can be easily and readily addressed in these ways. We do understand, though, that sometimes problems arise which cannot be addressed sufficiently in these ways.
